And when something blows up — a biased hiring filter, a pricing algorithm that craters margins in a key segment, a supply chain model that confidently optimizes into a single point of failure — leadership will point to the governance framework and say "we had controls in place." They did. The controls just didn't work because nobody was incentivized to actually use them. If you can answer this one question, I'll believe your organization is serious: Who gets fired when an AI-influenced decision goes catastrophically wrong — and did that person have genuine authority to prevent it? If you can't name that person today, your governance framework is decoration. Fix that first. Everything else is commentary.
